Does anyone have the chords or some form of sheet music to "Road to Calvary?"

Someone emailed me looking for it, but I don't think anything was ever officially put out.

I don't remember the song exactly... and I don't have it here at job.
but by what I remember by far....
at least the chorus....
(as I don't remember the tone of the song, I'll put it as grades...
