Where is the 'Contact Us' form??

Where is the 'Contact Us' form mentioned in the FAQ? Also, I am in the list of members but I cannot use the 'The Team' link and since I do not know where the contact us form is I cannot ask why it isn't working. If anyone can help with either of these questions it will be well appreciated

Not sure where the contact us form is. As for the team button, it doesn't work for me, it's probably disabled for everyone. Presumably it'd show you the list of moderators and administrators.
